Roofing Inspection Affidavit for deck nailing, underlayment, and flashing

Affidavit must be completed, dated after work begins, and uploaded to permit prior to the final inspection being scheduled. Failure to have affidavit completed at final inspection will result in a failed with fee result.

A General Building Residential Roofing Contractor Engineer Architect or Building Inspector as defined in Florida Statutes affirm that I did personally inspect work completed at this address for the following and that work is completed in accordance with all applicable Florida Building Codes Florida Statutes manufacturers installation instructions and permit documents submitted.

Based upon my examination, I have determined the installation was done in conformance with the requirements of FBC EB 706.7 Mitigation.
